AMA Defends Gender-Affirming Care Amidst Rising Political Tensions and Scrutiny

TL/DR –

The American Medical Association (AMA) has faced scrutiny over its support of gender-affirming care for transgender youth, with 20 Republican state attorneys general pushing the AMA to oppose such treatments for minors. They suggest that the AMA might be violating state consumer protection laws, potentially misleading medical providers and patients about their stance, and threaten a formal investigation. However, the AMA maintains that it has remained supportive of gender-affirming care, stating that while surgical intervention in minors generally should be deferred until adulthood, treatment decisions should be made between the physician, patient, and family based on medical evidence and clinical judgement.

US Medical Association and Transgender Youth Care Caught in Political Rhetoric and Healthcare Realities

The American Medical Association (AMA), the largest medical association in the US, finds itself in a complex situation around healthcare for transgender youth. Recent political discourse has created confusion and hostility, resulting in ambiguity for medical providers and families considering gender-affirming care.

There is growing uncertainty regarding the AMA’s stance following a January meeting with leaders of major medical groups and Dr. Mehmet Oz, the head of the Centers for Medicare and Medicaid Services. The American Society of Plastic Surgeons (ASPS), who attended the meeting, seemed to take a more restrictive stance on gender-affirming care, raising questions around the AMA’s position.

Twenty Republican state attorneys general are pushing for the AMA to oppose gender-affirming care for minors, citing concerns over potential violations of state consumer protection laws. A letter in February questioned whether the group recommends hormone therapy or puberty blockers to treat gender dysphoria in minors.

Khadijah Silver, director of gender justice and health equity at Lawyers for Good Government, argues that this is a political strategy to delegitimize gender-affirming care. Any perceived shift in the AMA’s stance could greatly impact public opinion.

Despite the AMA’s affirmation that their stance remains unchanged, the issue of gender-affirming care for trans youth constantly finds its way into political discussions, including a recent congressional hearing on rising healthcare costs.

The AMA supports evidence-based treatment for medical care, including gender-affirming care. They believe surgical interventions in minors should generally be deferred until adulthood. Similarly, the World Professional Association for Transgender Health (WPATH) recommends that patients wait until adulthood before seeking surgery.

Despite the controversy surrounding surgeries, few transgender youth undergo such procedures. Many depend on hormone therapy and puberty blockers to manage gender dysphoria, a medical condition causing significant distress for trans individuals.

Puberty blockers delay puberty’s hormones, which can be emotionally challenging for trans minors. Hormone therapy, involving testosterone or estrogen, can help align one’s physical appearance with their gender identity. Studies show these treatments can reduce depression and anxiety among trans youth.

However, despite their efficacy, these treatments are banned in many states due to political pressure.

The Endocrine Society and the American Academy of Pediatrics, both supporters of gender-affirming care, are under federal investigation over their stance. The attorneys’ general letter to the AMA seems to heighten this pressure on medical groups.
