Holistic Care Coordination (HCC) Expansion in Kansas

Holistic Care Coordination (HCC) is a comprehensive, patient-centered approach, which takes into account physical, emotional, and social aspects of patients and their families when devising health care plans. In 2023, the Kansas Department of Health and Environment’s (KDHE) Title V Maternal and Child Health Services Program joined forces with the Kansas Health Institute to investigate the possibilities of widening the HCC implementation in primary care and across maternal and child health programs in Kansas.

They have released a report that provides valuable insights into the existing HCC services for Kansas residents that are either pregnant, in the postpartum stage, or are children, primarily aged between 0-8. This report aims to guide future enhancements to HCC policy and practice in Kansas.
