
Hampton VA Health Care System Earns Four-Star Rating in CMS 2025 Ratings

Over the past year, the healthcare system has implemented improvements including daily tiered huddles, enhanced leader rounding, and the establishment of the Quality Assurance Performance Initiative Committee. As per the 2025 CMS ratings, 77% of VA hospitals received a four or five-star rating, with over 90% maintaining or improving their previous year’s rating.
Hampton VA Health Care System Earns Exceptional Rating on the CMS 2025 Hospital Quality Ratings
The Hampton VA Health Care System is celebrating its achievement of a four-star rating in the Centers for Medicare and Medicaid Services’ (CMS) 2025 hospital quality ratings. This ranking reflects the hospital’s commitment to providing exemplary patient care and service to veterans.
The CMS’s hospital ratings are assessed based on five significant categories: patient mortality, safety of care, readmission rates, patient experiences, and the timeliness and effectiveness of care. The higher the star rating, the better the hospital’s performance on these quality measures.
The Hampton VA has implemented several service improvements over the past year to better cater to veterans. This includes the institution of daily tiered huddles across the enterprise, watch-list huddles for at-risk veterans, structured and standardized shift reporting, enhanced leader rounding, and the establishment of the Quality Assurance Performance Initiative (QAPI) Committee.

In the 2025 CMS ratings, 77% of VA hospitals receiving an Overall Hospital Quality Star Rating achieved 4 or 5 stars, with no VA hospitals receiving a one-star rating. Over 90% of rated VA hospitals have maintained or improved their 2024-star rating. You can view the star ratings online.
